Lost Passport Procedures
If you have lost your passport, first of all, it is very necessary to report the loss of your valid passport to the nearest police. Then, you should visit the local U.S. Embassy. There, you will need to submit an application form for replacement of your passport. |
To report the loss of your valid passport, you will be required to fill the form DS-64. You should personally go to the U.S. Embassy office and complete all the procedures; alternatively, you can also mail the form DS-64 to the U.S. Department of States, Passport Services but it is advisable to complete the procedure in person.
After completing the form DS-64, you should complete the application of new passport by filling the form DS-11. You will be required to submit documents such as proof of your citizenship i.e. your birth certificate, and identity of proof i.e. you’re driving license.
After you have completed filling all your application forms, it is sent to the Embassy of the United States for processing. It may take few weeks of time to get the replacement of your lost passport as it requires complete verification of the details provided by you for the loss and the issuance of a new one. However, if you’re in a hurry to travel for any critical reason and you do not have time to wait for a full-validity passport, you can request for an emergency passport. Such emergency passports act as a provisional passport and are valid only for twelve months. Moreover, emergency passports cannot be extended further.
